PharmaHealth’s Clinical Pharmacists are available to assist physicians, nurses, house managers, patients and other healthcare professionals with any questions or concerns regarding a patient’s medications.
How Can Our Clinical Pharmacists Assist You?
- Maintains liaison relationships with medical and nursing staff.
- Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes.
- Advises physicians on issues concerning drug therapy, the inherent toxicity of drugs and their immediate and long–term side effects, as well as assists in the prescription of appropriate doses.
- Conducts and evaluates medication histories, suggests drug therapies and monitors patient responses and reactions to medications.
- Assess compliance and suggests modifications so as to achieve desired outcomes and evaluates patient progress.
- Individualizes medication regimens using sound principles, accounting for variations in drug absorption, distribution, metabolism and elimination.
- Instructs patients in the proper use of their prescribed medications to prevent excessive usage or harmful interactions.
- Advises and checks the work of the pharmacy technicians to make sure that all dispensed drugs are recorded and delivered in accordance with the physicians’ prescriptions as well as local, state and federal laws and regulations.
